Where are you located?
We are located at 5157 N. Francisco Ave in the same building as the Galter LifeCenter, but we have a dedicated entrance on the south side of the building.
What are your hours of operation?
We have appointments available Monday through Friday 6:30 am to 8 pm.
Do I need a referral from my doctor?
Yes. We require a physician's order for therapy, including the reason your physician is referring you to physical therapy.
Does my insurance pay for therapy?
Physical Therapy is usually covered by most insurance plans, but individual plans do differ significantly. We strongly advise checking with your insurance company.
Is parking available?
Free spaces are available in the parking lot. If a spot is not available, we also have a large parking structure with ample parking for a minimal charge. There is also street parking available on Foster Ave. and Francisco Ave.
How long are the sessions?
Your session will be approximately 45 minutes with one therapist.
How will my doctor know what progress I have made in therapy?
After your first visit, your referring physician will receive a summary outlining the plan of care that you and your therapist have agreed on, and your physician will receive updates at regular intervals documenting your progress.
