What to bring to your child's appointment
To help Dr. Sagan and Dr. Vijay become familiar with your child's medical history, it is helpful to bring copies of records to the first appointment, if possible. Some examples of helpful information are:
- Growth curves
- Screening records (such as lead testing)
- Vaccine records
However, if your child is medically complex, has specific health issues or takes chronic medication, Dr. Sagan and Dr. Vijay will need the most recent consultant notes from their current pediatrician.
Transferring your child's medical records
If you would like Dr. Sagan or Dr. Vijay to be your child's pediatrician at the NorthSide Pediatrics office, please forward your child's medical records to NorthSide Pediatrics.

To obtain these records you will need to fill out request forms, either from your current pediatrics office or Northside Pediatrics. (Many practices charge a fee for copying these records, so you may have to pay your current pediatrics office.) The documents will be sent to Dr. Sagan's new NorthSide Pediatrics location at Swedish Covenant Hospital.
To help you save costs on transferring records, Dr. Sagan recommends only requesting screenings (such as lead testing), growth curves and vaccine records — unless your child is medically complex, in which case Dr. Sagan and Dr. Vijay needs the most recent consultant notes.

Parking
Parking is available for visitors in the parking garage located on California Avenue. The garage is open 24 hours a day, 7 days a week and is monitored by Swedish Covenant Hospital Public Safety personnel. Handicapped parking spaces are available. The visitor parking fee is $4.00 per day.
Driving Directions
From the northern suburbs
Take I-94 eastbound (Edens Expressway) to the Cicero/Foster Avenue exit. Follow Cicero Avenue south to Foster, turn left on Foster and proceed east approximately 2 miles to California Avenue. Turn right in California to reach the Hospital's Main Entrance and parking garage.
From the south
From the southwest suburbs or Midway Airport, take Cicero Avenue north to I-55 (Stevenson Expressway). Follow signs to I-90/94 West (toward Milwaukee, O'Hare Airport, or Rockford). Do not use the Express Lanes. Exit at Kimball Avenue. Turn right on Kimball, and proceed north two miles to Foster Avenue and turn right. Proceed one mile to California Avenue. Turn right on California to reach the Hospital Main Entrance and parking garage.
From the northwest
From 294 (Tri-State Tollway) or O'Hare Airport, proceed to 90 East. Take I-90/94 (Kennedy) eastbound to the Austin/Foster Avenue exit. At the end of the exit ramp, turn left and follow the curve to reach Foster. Turn left on Foster and continue to California Avenue. Turn right on California to reach the Hospital Main Entrance and parking garage.
From downtown Chicago or lakeshore suburbs
Take Lake Shore Drive (from either north or south) to the Foster Avenue exit. Travel west on Foster for approximately two miles to California Avenue. Turn left on California to reach the Hospital Main Entrance and parking garage.

Public Transportation
Bus
- The Chicago Transit Authority (CTA) bus route #92 Foster runs along Foster Avenue past the Hospital. Get off at California Avenue to reach the Hospital Main Entrance.
- The CTA bus route #93 North California runs along California Avenue past the Hospital. Get off at Foster Avenue to reach the Hospital Main Entrance.
Train
- From the CTA Brown Line train Kimball station, you may transfer to bus route #93 North California at the station. Get off at Foster Avenue to reach the Hospital Main Entrance.
- From the CTA Red Line train Berwyn station, you may transfer to bus route #92 Foster at the station. Get off at California Avenue to reach the Hospital Main Entrance.
- From the Metra Union Pacific North Line, get off at the Ravenswood station (Lawrence Avenue). Take the westbound #81 Lawrence bus, and get off at California Avenue. From there, take the northbound #93 North California bus to Foster Avenue or walk six blocks to the Hospital.
