Care and Services Available at Home
Swedish Covenant Hospital's Home Health Care team is prepared to make a first visit to your home within 48 hours of receiving an order and referral from your doctor.
Our team of registered nurses on staff is ready to help with:
- Physical assessment and care coordination tailored to address your medical needs
- Patient and family/caregiver education
- Education and support re disease, wound care and pain management
- Education and assistance re administration of injections/intravenous medications
- Management and education of medications at home
- The use of equipment, treatments and technologies that improve your ability to manage your condition at home like woundvac, treemonitoring and tube feeding
- Collaboration with your doctor
- Assistance with appointments and referrals to other services/resources as needed
Our home health aides:
- Provide support services under the supervision of the registered nurse or therapist when needed as long as skilled care continues
- Assist with personal care such as bathing, grooming and dressing
Our skilled therapy team on staff includes physical therapists, occupational therapists and speech language pathologists prepared to assist with:
- Assessment of your home environment and your current level of function
- Recommendations/suggestions for durable medical equipment and adaptations that might be needed to help you function safely in your home
- Development of a treatment plan and goals designed to meet your specific needs
- Training on the use of special equipment and the safe performance of daily activities
- Speech therapy to aid in regaining and strengthening of speech, memory, swallowing and/or cognitive skills
- Home exercise programs tailored for you and the environment you live in and designed to help you regain movement and strength
- Hands-on therapy/treatment sessions
- Ongoing evaluation and assessment of your progress towards goals and independence
- Fall prevention and recovery education
Our medical social worker is available to:
- Help with social and emotional concerns related to illness
- Facilitate communication between patient, family and/or caregiver
- Provide referral to community resources
- Assist with management of medical bills and financial concerns
